Employment and sports physicals are limited, form-based exams. They are paid by you or your employer, are not billed to insurance, and do not replace a full annual wellness exam.

Your PJM provider orders labs, and Sonora Quest Laboratories performs them. Self-pay patients pay Sonora Quest directly at its published discounted pay-at-time-of-service (PATS) rate, with no PJM markup. Sample prices below are from Sonora Quest’s PATS fee schedule dated March 4, 2026; Sonora Quest sets these prices and may change them.

Any Sonora Quest collection fees are set by Sonora Quest. In-office rapid tests will be added after our CLIA certificate is on file.
Membership is an optional monthly plan for adults 18 and older. Self-pay only, no insurance billed. Pay monthly by cash or card (card on file), your choice. Plans effective October 11, 2026.
Not included in either plan: lab tests, medications (including the drug for injections), imaging, and outside specialists. These are paid separately.
Insurance plans are being added. If you have Medicare or AHCCCS, please tell us first; we will explain your options before any service.
PJM Clinic provides family primary care, including geriatric-focused care, physical health services, and lab testing, with referrals to outside specialists. Outside lab, specialist, and pharmacy charges are separate. We do not provide urgent or emergency care. For emergencies, call 911.
